The best AI voice agents for pharmacies come in two kinds, plus one you run yourself. Dograh, an open-source, self-hostable voice agent platform, runs on the pharmacy's own servers and reads and writes the pharmacy management system during the call. Specialists such as Lumistry, Pharmesol and VOXO come wired to the PMS; Retell AI and Vapi are hosted.
Key Takeaways
- Choose by the PMS you run and where the recordings must live.
- Dograh runs on your own servers, with no per-minute platform fee when self-hosted.
- Test the pharmacist handoff and the verification check on a real phone line.
This post is part of our guide to AI voice agents for pharmacy and pharma. The guide's section "Choosing a pharmacy voice agent, and where to start" says the choice turns on which PMS you run and where you want the recordings kept. Here are eighteen products, judged on the five tests that matter to a pharmacy, starting with Dograh.
The five tests a pharmacy buyer should run
Pharmacy phone lines are busier per counter than they were. In Drug Channels, Bryce Platt notes that more than 2,000 closed in 2025 alone, so the same prescriptions reach fewer stores. These five tests decide whether an agent takes calls off that counter safely.
The first test is where calls and recordings run, on the pharmacy's own servers or only in the vendor's cloud, and which plan allows each. The second is the pharmacy management system (PMS). The agent should read status, refills and copays from it during the call and write requests back. A product already wired to your PMS saves build time, while a product your team connects gives you control.
The third is the business associate agreement (BAA). Under 45 CFR 160.103, any vendor that "creates, receives, maintains, or transmits protected health information" for you is a business associate. What matters is which plan carries the BAA and which parts of the call it covers.
The fourth is the handoff. Every question about the medicine itself should reach a pharmacist, and a medical emergency should hear one fixed line: hang up and call 911.
The fifth is price, meaning what a minute costs at your call volume and what sits on top, such as models, phone minutes and add-ons.

Dograh: open source you run on pharmacy servers
Dograh is a full voice agent platform whose code a pharmacy can read and run on its own servers without a sales contract. It is released under the BSD 2-Clause licence and has more than 5,900 stars on GitHub.
That choice decides who hears the call. With self-hosting, recordings and transcripts stay on your servers. Dograh is the orchestration layer: it runs the call flow, tools and telephony, and the speech and language models are a separate layer you choose. The call audio stays in-house only when the speech and language models run on your own servers too. For patient calls, self-host Dograh. No BAA with Dograh is needed, because Dograh never sees the data. Your speech and language providers may need their own BAA, and your phone carrier may too.
Dograh starts every account on its own managed models. To keep call audio away from Dograh, open Models, switch the Organization default to BYOK with your own speech and language providers (or local models), and check that no agent has its own model override.
For the PMS, HTTP API tools read an order's status, refills left or the copay on file during the call and write the request back. Pre-Call Data Fetch can load the caller's open order as the call connects, and several store numbers can point at one agent. Every answer comes from your system after the caller passes an approved check.
The safety lines are fixed words. The emergency pathway's Transition Speech plays "hang up and call 911" exactly as written. On a speech-to-speech model only recordings play, so record it. Transfer Call hands medicine questions to the pharmacist. Transfer Call works on Twilio, Telnyx and Asterisk ARI. It passes no summary by default; on Twilio it can play a short spoken introduction to both people. Write the details to your system with an HTTP API tool before the transfer.
Self-hosted Dograh runs 10 concurrent calls by default, raised with a setting, and your servers and carrier set the real ceiling. Self-hosted Dograh has no per-minute platform fee (servers, model keys and phone minutes still cost); Dograh Cloud has a 1¢-a-minute platform fee, and Dograh's pricing page lists Enterprise on-premise or private-cloud deployment.
Your team connects the PMS through its own API, and texts, payments and the opt-out list stay in your own systems, which keeps patient records in one place.

Pharmacy specialists: PMS connections built in
Pharmacy specialists come with the PMS connection and pharmacy rules already built, and each is bought through a demo. They suit a pharmacy that wants a working product more than control over where it runs.

Lumistry, a pharmacy software company, names Liberty, BestRx, ComputerRx and Rx30 among the systems it works with, the clearest PMS list in the set. Pharmesol works across patients, providers, pharmacy benefit managers (PBMs) and other pharmacies, and says it reads and writes directly into the PMS. VOXO sells a pharmacy phone system with the agent on top, so it also carries the call.
Asepha isolates each site from the others, and Pharmie AI, a Y Combinator company, answers inbound calls around the clock. PolyAI sells enterprise agents across industries, and its healthcare page lists specialty pharmacy work such as confirming refill readiness and scheduling deliveries. For all six, ask the same three things: which PMS versions the connection covers, where recordings are stored, and which contract carries the BAA.
General voice platforms with a BAA option
These sell to every industry. Your team builds the pharmacy flow and the PMS connection, and each puts its healthcare terms on a different plan, so read the plan, not the badge. Retell AI's compliance page states that "A signed BAA is required before transmitting PHI through Retell". Vapi bills models and phone on top of its hosting rate, while Bland AI's rate includes the model and both speech layers. Thoughtly and Smallest AI put their HIPAA terms on Enterprise or an add-on.
LiveKit and Pipecat are open-source frameworks rather than finished products: your developers write the agent in code and run it on your servers or on the vendor's cloud. Air AI is in the table because buyers ask about it. The FTC sued it in August 2025 over deceptive claims about earnings and refunds, so check that case before any talks.
Retell AI, Vapi and Bland AI also appear in our roundup of the best AI voice agents for healthcare. For a pharmacy, the difference from Dograh is where the result runs.
What every product must prove in a trial
Whatever you shortlist, run the same tests on a real phone line before any patient hears the agent. Call as a relative and check that no medicine is named before the check. Ask "should I stop taking it?" and check the call reaches the pharmacist with the question written to your system.
Then say you are having chest pain and listen for the fixed emergency line. Have the agent read a test order from your PMS and write a refill request back. Say "stop calling me" and check the opt-out lands in your own list.
Watch outbound offers too. Under the FCC's healthcare call exemption, calls "must not include any telemarketing, solicitation, or advertising", so an offer of vaccines or delivery generally takes an outbound call outside it. Keep offers off those calls, and confirm your position with counsel. AI voice agents for pharmacy recall notifications shows those limits on a real call type.
Testing Voice Agents Before You Go Live sets the order of tests, and HIPAA-compliant voice AI for pharmacy covers what to ask about every vendor that holds the call.

Which product fits which pharmacy
A pharmacy or chain that wants patient calls and recordings on its own servers should start with Dograh self-hosted. It is the strongest fit for that need, because the code is open, there is no per-minute platform fee when self-hosted, any model can run including local ones, and the agent reads and writes your PMS during the call in a flow your team controls.
An independent pharmacy on Liberty, BestRx, ComputerRx or Rx30 that wants a ready product will look at Lumistry first, and at Pharmesol, VOXO, Asepha or Pharmie AI for outbound calls, SMS or a new phone system. A large specialty pharmacy may add PolyAI. A developer team that wants a hosted platform this month can start on Retell AI, Vapi or Bland AI, and a team that wants to write everything in code can use LiveKit or Pipecat.
To try Dograh, self-host it from GitHub with Docker Compose, or build the flow on Dograh Cloud with test data only. Point one test number at an order-status call that reads a test PMS record, then move to patient calls on your own servers, where the code, the recordings and the PMS connection are all yours.
Glossary
- Pharmacy management system (PMS)
- The software a pharmacy uses to record prescriptions, fills, prices and patients, such as Liberty, BestRx, ComputerRx or Rx30. A voice agent reads status, refills and copays from it during the call and writes requests back.
- Business associate agreement (BAA)
- The contract HIPAA requires with a vendor that creates, receives, maintains or transmits protected health information for a pharmacy. It delegates obligations to the vendor and never transfers the pharmacy's liability.
- Self-hosting
- Running the voice agent platform on servers the pharmacy controls. With open-weight models on the same servers, call audio and transcripts stay in-house.
- Concurrent calls
- How many calls a platform runs at once. Self-hosted Dograh starts at 10 and can be raised; hosted plans include a set number, such as 20 on Retell AI or 4 on Vapi's base tier.
